Welcome to Our Practice

We would like to take this time to thank you for choosing Whidbey Vision Care for all of your eye care needs.  Whidbey Vision Care strives to provide its patients with state-of-the-art eye care, whether we are seeing you for your routine eye exam or managing and treating eye related injuries, infections or diseases.  Our physicians practice to the full extent of their licenses and are highly skilled and knowledgeable, keeping abreast of the newest diagnostic methods and treatments. 

Our technicians, optical staff and front office staff are also highly skilled and knowledgeable.  They are readily available to answer your health care questions, to help you understand your insurance benefits, and to assist you with your eyewear selections.

Below you will find printable pdf's of our New Patient Information form, the HIPPA Disclosure form and a Review of Systems form.  If you prefer to complete these documents prior to your appointment you may print them and bring with you. If you have a list of medications that you are currently taking, it would be helpful to bring a copy for the doctor.  Also, plan to have your insurance card(s) available so that we may make copies for your file. 

As a courtesy to our patients, we contact your insurance prior to your appointment for a complete benefit outline.  If your insurance assesses a co-payment, we are required to collect it at the time of service.  If you have any questions regarding your coverage, our office staff will be happy to go over the benefits quote with you.
